Does it include the start and end days?

The difference is the number of full days between the dates. If you count "today" as day 1, add 1 to the total.

How are months calculated?

Months are an estimate using the average 30.44 days per month, so they're a guide, not an exact calendar month count.

What if the target date has passed?

The tool shows the number of days since — just note whether the result is "until" or "since" in the output.

Does it work across time zones?

Dates are compared by calendar day, so time-zone differences don't affect the count.
